I was reading over this and just for the good of the community, I am running 64-bit kernel 4.11 and I experienced the same thing. Putting everything into Legacy boot mode worked.

Yes. Also, the smb portion works fine from a PC over PXE boot during the CD workflow.

Here's another issue. I can netboot into the nbi that I created. I log into CloneDeploy and start On Demand. It fails during the mounting of the smb share.
Error: Could Not Mount SMB Share: mount_smbfs: server connection failed: No route to host

If I cancel the terminal windows and mount is using the gui (Go, Connect to Server) it mounts fine.
If I open another terminal window and type the following, it also mounts fine (after entering the password).
    mkdir /Volumes/storage mount_smbfs //'WORKGROUP;cd_share_rw'@10.1.2.55/cd_share /Volumes/storage

I made a new nbi from a new autodmg image and now it works. Thanks!

Here is the kernel panic log if it tells you anything. I'm going to recreate the nbi from scratch and test again.

Created it from 10.11.6. I included updates in the autodmg window. Not sure if that makes a difference.
Test machine is MacBook Pro Retina 2015.

Yes, that works. I can download the dmg file. I have replaced the IP (I didn't copy it correctly into the forum).

Right, I corrected the apple-root-path to have the correct IP of 10.1.2.55. I recopied the nbi dmg and i386 data to their respective locations.

    CloneDeploy - Windows Server 2012 R2
    Proxy - Ubuntu Server 16.04

I'm still getting the kernel panic, though.
    Here is the corrected config.ini
